Feel free to download and share these forms with all participants. Remember, all medicals and releases must be completed and signed appropriately before participation in Sierra Expeditions activities. All of our forms are in Adobe Portable Document Format.

  • Leaders Guide
    • This document holds just about all of the information you should need before participating in any of our programs.
  • Class 1 Medical Form
    • All Participants must have this form completed and signed to participate in Sierra Expeditions.
  • Class 3 Medical Form
    • Due to the strenuous nature of Sierra Expeditions activities, all participants must also have the Class 3 Medical Form as well.
  • Parental permission, and photo release
    • This form is to permit us to include photos of your unit or individuals as part of our publications and on our website.
